Who do we support?
We support individuals who are leaving prison with co-occuring needs; individuals who are / or are at risk of homelessness who have a history of offending, mental health or substance use issues. Individuals must be aged 25+ and be in need of intensive, housing support. They must be registered with Carmarthenshire Local Authority.
What is the purpose of this service?
We provide supported accommodation to clients in a shared community housing setting across multiple properties in Llanelli, and access to flexible support from a dedicated Support Worker.
We liaise with Probation and Integrated Offender Management, utilise a trauma informed and psychologically infomed approach to create support plans for clients to improve skills and prepare them for independent living, as well as preventing re-offending.
What support does your service provide?
- Managing money, benefits, arrears and bills
- Managing accommodation including daily living skills
- Liaising with other services such as Probation, Mental Health, and Drug & Alcohol services
- Access to our dedicated Solution Focus Service.
- Integrate into the community – working with other agencies to join groups, find hobbies and socialise
- Find volunteering or paid work
- Support to help someone secure accommodation and move on successfully
- Any other support needs identified by the client
